ADDITIONAL COMMENTS
1. The inspection was conducted at Reliable Onsite Services, Gastonia, NC.
2. The following firm truck was inspected:
Vehicle Identification Number (VIN) NC Tag Number Capacity (gallons)
5PVNJ8JV1K4S72807 JE-7468 1100 waste/500 fresh
3. The tank appeared to be structurally sound and bolted to the frames.
4. Valves and access ports appeared to be secure. Caps were present on the access ports.
5. The truck was properly marked in accordance with the lettering requirements.
6. No leaks were observed.

such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
