The permit to operate shall expire July 8, 2019. Pursuant to 15A NCAC 13B .0201(g), no later than January 8, 2019, the permittee must submit a request to Section for a permit amendment and must update pertinent facility plans including, but not limited to, the facility plan, operation plan and waste screening plan. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance inspection. STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS: 01 - Facility has not submitted the request to the DWM-SWS for a permit amendment as of today. John College spoke on the phone with Angel White – Landfill Manager about expediting this request to the SWS ASAP. 02 - This facility has a locking Gate at roadside entrance for security. 03 - Signage has required information. 04 - Facility is permitted to receive waste from Perquimans, Chowan and Gates Counties. 05 - Waste must be transported to The East Carolina Regional Landfill (0803-MSWLF-1993) in Bertie County. 06 - Permit and Contingency Plan were reviewed. 07 - John College made several suggestions to update this facilities Contingency Plan. 08 - Incoming load tickets were reviewed. 09 - Screening records were reviewed and were in compliance. 10 - Facility has 3 Certified Transfer Station Operations Specialist: Charlie Rome: expires 3/12/2019, David Lowe: expires 5/25/2019 and James Bolen: expires 10/9/2020. 11 - Onsite operator during this inspection was Charlie Rome. 12 - Scrap tires are being loaded into open top box trailers. 13 - White goods containing CFC’s are now being kept upright. Did not review records of CFC removal during this inspection. 14 - Scrap metal area is also being managed better. Pile is starting to get very large. 15 - Pesticide containers are being collected at this site and recently been removed. 16 - Electronics are being collected at this site. Packaged for removal. 17 - Windblown trash management is being done well. 18 - Need to update 3 ring binder in scale house with the addition of recent inspection reports and recent annual reports. 19 - You may want to add the LCID, White Goods and any other miscellaneous Solid Waste managed at this site to the new transfer station permit. You should speak with the DWM-SWS Engineer (Ethan Caldwell) that will be working on your permit about these possible additions. 19 - Leachate system is working well. 21 - Tipping floor clean. 22 - Overall management of this facility is good. 23 - Digital pictures were taken during this inspection.
