administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected second vehicle to be added to firm permit. Tag#YA151104, VIN 1HSHJAZN1PH514950, 2500
gallons. The appropriate fee of $800 was paid to the Raleigh Central office for two permitted vehicles in 2019.
2. The tank appears structurally sound & securely attached. The outlet covers were on and no drips/leaks were
noted. The lettering is correct on both sides of the vehicle, however the decals for the phone number & town
name were damaged on both sides of the truck (will need replaced/repaired in the near future). This vehicle
is in compliance at this time.
3. The other permitted vehicle was not inspected at this time and truck logs were not reviewed.
4. Enclosed is a 2019 class schedule for your use. Please register for one of the class locations.
