A closure soil sample was collected from immediately beneath the UST. Concentrations of TPH-GRO and TPH-DRO that exceeded the RALs were reported in the closure sample. Based on field observations, limited over -excavation was conducted. Upon completion of excavation activities, one soil sample was collected from the base of the excavation and four soil samples from the sidewalls of the excavation. Concentrations of one or more BTEX constituents, naphthalene, MTBE, n-butylbenzene, ispropylbenzene, n-propylbenzene, 1,3,5-trimethylbenzene, 1,2,4-trimethylbenzene, acenapthene, 2- methylnapthalene, C5-C8 aliphatics, C9-C18 aliphatics, C9-C10 aromatics, and Cl 1-C22 aromatics that exceeded the soil to water MSCCs were reported in all five excavation samples. SITE HISTORY AND CHARACTERIZATION 1. UST Owner and Operator Information: In accordance with the Initial Abatement Action (IAA) Report guidelines, the UST owner and operator information has been summarized in Table 1. 2. UST System Information: In accordance with the IAA Report guidelines, the UST system information has been summarized in Table 2. 3. Non -UST Information: Not applicable. 4. Comprehensive Description of the Release: On November 4, 2014, the 550-gallon heating oil UST was removed. Based on field observations, a release was suspected at the site. A 24-Hour Release and UST Leak Reporting Form (UST-61 Form) was submitted to NCDENR on November 11, 2014. A copy of the UST-61 Form is included as Appendix A. 5. Description of the Site Characteristics: The Rock Worship Center is an active religious facility located at 1113 Fordham Road, in Charlotte, Mecklenburg County, North Carolina (Figure 1). The adjoining properties are residential. Public water is available to the site and surrounding properties. 6. Summary of Initial Abatement Actions, Assessment Activities, and Corrective Actions: The UST was a non-commercial, non -regulated heating oil tank. The 550-gallon heating oil UST was removed from the site on November 4, 2014. Based on field observations, limited excavation activities were conducted. Excavation was limited due to the presence of the church building and sidewalks. B. SITE CHECK A site check was not part of the initial abatement activities. C. UST CLOSURE 1. Preparation for Closure: A Charlotte Fire Department UST removal permit was obtained. Prior to closure, Charlotte Fire Inspector Kevin Miller was notified. A copy of the permit application was provided in the 20-Day report submitted on November 13, 2014. 2. Closure Procedure: On November 4, 2014, one 550-gallon UST was purged of approximately 50 gallons of fluids and subsequently removed. The UST was in poor condition with some small holes observed. The dimensions 1 of the UST were 48 inches in diameter by 6 feet in length. The UST was transported off -site and properly disposed of. Information and manifests pertaining to the removed fluids, disposal of the UST and contaminated soil were included in the 20-Day report One closure soil sample (UST-1 (7')) was collected from beneath the UST and submitted for laboratory analyses of total petroleum hydrocarbons -gasoline range organics and diesel range organics (TPH- GRO/DRO). A sample was also collected from the stockpiled soil for analysis for TPH-DRO/GRO. Concentrations of TPH-GRO and TPH-DRO reported in the closure sample exceeded the regulatory action levels (RALs). Concentrations of TPH-GRO and TPH-DRO above the RAL were also reported in the sample collected from the stockpiled soil. 3. Residual Material: Approximately 50 gallons of fluids were pumped from the UST prior to removal. 4. Release Information: A UST-61 Form was submitted to NCDENR on November 11, 2014. A copy of the UST-61 form is presented in Appendix A. 5. Soil Excavation Activities: Based on field observations, limited soil excavation activities were conducted (Figure 2). A total of 18.82 tons of contaminated soil was transported to Environmental Soils, Inc., a permitted disposal facility in Lattimore, Cleveland County, North Carolina, 6. Site Investigation: Upon completion of excavation activities, GRI collected one confirmation soil sample (CS-5-10') from the base of the excavation and four soil samples (CS-1-10', CS-2-10', CS-3-10' and CS-4-10') from the sidewalls of the excavation. The confirmation soil samples were analyzed for volatile organic compounds (VOCs) using EPA Method 8260B, semi-VOCs using EPA Method 8270, and MADEP Methods EPH and VPH. The dimensions of the excavation and soil sample locations are included on the Site Map presented as Figure 2. No ground water or bedrock was encountered during excavation activities at the site. A summary of soil sample analytical results is presented as Table 3. A copy of the laboratory analytical report has been included as Appendix B. 2 D. FREE PRODUCT INVESTIGATION AND RECOVERY REPORT No free product was observed beneath the UST during closure activities. E. GROUNDWATER AND SURFACE WATER INVESTIGATION Ground water and surface water investigation was not conducted as part of the initial abatement actions. F. INITIAL RESPONSE AND ABATEMENT ACTION Initial Response Actions: The 550-gallon heating oil UST was removed from the site on November 4, 2014. Based on field observations, over -excavation activities were continued on the UST basin. Excavation was limited due to the proximity of the church building and sidewalks. The excavation was baekfilled to grade with clean fill material. A 20-Day Report was previously submitted on November 12, 2014. • Source Control Actions: The UST system was removed on November 4, 2014. • Contaminant Migration Control Measures: The release was to the sub -surface; therefore no contaminant migration control measures were necessary. • Measures Taken to Mitigate Fire/Safety Hazards: There are not any immediate fire or safety hazards present as a result of the release. • Measures Taken to Identify and Mitigate Pollution Hazards: The release was to the sub -surface; therefore, no migration control measures were necessary. 2. Initial Abatement Actions: • Contaminated Soil Storage/Treatment and/or Disposal: A total of 18.82 tons of contaminated soil was transported to Environmental Soil, Inc., a permitted disposal facility in Cleveland County, North Carolina. G. EXCAVATION OF CONTAMINATED SOIL 1. Extent of Soil Contamination: Based on the observations from GRI staff, excavation activities were conducted. A total of 18.82 tons of contaminated soil was removed. The final UST basin over -excavation dimensions were approximately 9 feet long by 10 feet wide and 10 feet deep. No free product or ground water was encountered in the excavation. Contaminated soil was transported to a permitted disposal facility. 2. Excavation Process: The excavation activities were conducted using a track hoe. 3 3. Sampling Description: All soil samples were collected directly from the bucket of the excavator by GRI personnel. Samples were collected with clean, disposable nitrile gloves, immediately placed in laboratory -supplied glassware and placed on ice. 4. Sampling Results: Concentrations of one or more of BTEX constituents, naphthalene, MTBE, n-butylbenzene, ispropylbenzene, n-propylbenzene, 1,3,5-trimethylbenzene, 1,2,4-trimethylbenzene, acenapthene, 2- methy1napthalene, C5-C8 aliphatics, C9-C18 aliphatics, C9-C10 aromatics, and Cll-C22 aromatics that exceeded the Soil to Water Maximum Soil Contaminant Concentrations (MSCCs) specified in T15A NCAC 2L.0202 were reported in CS-1-10', CS-2-10', CS-3-10', CS-4-10' and CS-5-10'. The concentrations of C9-C18 aliphatics, C9-C10 aromatics, and Cll-C22 aromatics in CS-1-10', CS-2- 10', CS-3-10'and CS-4-10' exceeded the Residential Soil Cleanup Levels. A summary of soil sample analytical results is presented as Table 3. A copy of the laboratory analytical report is included in Appendix B. 5. Soil Disposal: According to the disposal manifests, a total of 18.82 tons of overburden and contaminated soil were loaded and transported to Environmental Soils, Inc. H. SUMMARY AND CONCLUSIONS • On November 4, 2014, one 550-gallon, non -regulated heating oil UST was removed. • The site is located in a residential area. Public water is available to the site and surrounding properties. • Based on field observations, over excavation was conducted. A total of 18.82 tons of contaminated soil was transported to a permitted disposal facility. • Neither ground water nor free product was encountered in the excavation. • Concentrations of one or more of BTEX constituents, naphthalene, MTBE, n-butylbenzene, ispropylbenzene, n-propylbenzene, 1,3,5-trimethylbenzene, 1,2,4-trimethylbenzene, acenapthene, 2- methylnapthalene, C5-C8 aliphatics, and C9-C10 aromatics that exceeded the MSCC specified in T15A NCAC 2L.0202 were reported in the excavation samples. The concentrations of C9-C18 aliphatics, C9-C10 aromatics, and Cl 1-C22 aromatics in CS-1-10', CS-2-10', CS-3-10'and CS-4-10' exceeded the Residential Soil Cleanup Levels. 4 I. LIMITATIONS This report has been prepared for the exclusive use of the Pastor Frank Jacobs for the specific application to the referenced site in Mecklenburg County, North Carolina. The assessment was conducted based on the scope of work and level of effort specified by the NCDENR and with resources adequate only for that scope of work. Our findings have been developed in accordance with generally accepted standards of geology and hydrogeology practices in the State of North Carolina, available information and our professional judgment. No other warranty is expressed or implied. The data that are presented in this report are indicative of conditions that existed at the precise locations sampled and at the time the samples were collected. In addition, the data obtained from samples would be interpreted as being meaningful with respect to parameters indicated in the laboratory report.
