1. The inspection was conducted at the westbound US 74, Haywood County Rest Area.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

3. This vehicle was damaged in an accident on US 74 in Lake Junaluska, Haywood County on October 15, 2018.
The Solid Waste Section was notified by telephone on the date of the accident. The remaining waste in the
vehicle was removed by another septage company. The company was instructed to apply lime to the area affected on the ground. Environmental impacts were investigated by the NC DWQ and Haywood County
Health Department. This is also a new pumper vehicle inspection. 4. The tank appeared to be structurally sound and bolted to the frame.
5. The valve and endcap were damaged in the accident and had been repaired. The valves and access ports appeared to be secure. Caps were present on the access ports.
6. The truck was properly marked in accordance with the lettering requirements.
7. No leaks were observed.
