The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Soil samples were required to be taken by the permit holder by December 31, 2018. At this time it does
not appear that this office has received a copy of the soil report yet.
2. Noted scattered thin patches of fescue on Field 2 – fescue appeared better established on Field 1. Continue
efforts to improve the cropstand & control weeds/vegetation other than fescue. Noted scattered ruts on
Field 1 - land application shall not occur during periods of high soil moisture or when the soil surface is
frozen or snow covered.
3. Field markers appear accurate on both fields. It appears that approved setbacks are being met but were not
measured at this time. No sign of land application outside the permitted boundaries.
4. No sign of heavy or recent application. No odors noted.
5. The required signs are posted and the access road is adequate.
6. The permit submittal is currently under review.
