STATUS OF PERMIT: Active in system, paperwork indicates that site was closed by the Section in 05/97. Requesting site status change to: CLOSED PURPOSE OF AUDIT: Closure Audit NOTICE OF VIOLATION(S): None You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Statute or Regulations. For the violation(s) noted here, you may be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE AUDIT REPORT Division of Waste Management Solid Waste Section Page 2 of 2 AREAS OF CONCERN AND COMMENTS: 1. First inspected site on 08/05/08. DOT representative on site to attempt to locate waste area. Site was fully vegetated, appeared stable, and the history indicated that it was created by DOT for LCID waste generated from the widening of Macedonia Road in 1996. 08/05/08 photo. It appears as though the LCID boundary starts at the `dip' in land. Old/unused vehicle and equipment were located on this area — but the site had full vegetative cover. The backside of the slope was steep, but stable and covered with established vegetation. 2. Site is inactive and paperwork indicated that a closure inspection was conducted by Janet Cantwell (SWS) on 05/13/97 during which the cap thickness was verified. At that time, the site had been "seeded, sloped, and a minimum of 1 foot of dirt had been placed on the top and side slopes" of the waste area. 3. A letter dated 05/20/97 from the DOT Division maintenance engineer (J.J. Swain, Jr.) requested closure, stating that "the subject landfill slopes were seeded and mulched and the remaining portions of the landfill will be seeded by 05/30/97." 4. Site visit on 06/02/09 verified property location, vegetation, usage. It appears as though no additional waste activities occurred following the 1997 closure of the LCID by the DOT. Recommending that the status of this site be changed to "closed" based on the documentation from 1997. Please contact me if you have any questions or concerns regarding this audit report.
