OBSERVED VIOLATIONS: None. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 4 ADDITIONAL COMMENTS This facility is permitted to receive solid waste generated by or at the Dan River Steam Station. Waste types include, but are not limited to: coal combustion residuals, wastewater treatment sludge, vacuum truck waste, waste soils (i.e. from ash removal activities or soil that has contacted petroleum), and small quantities of inert debris from CCR unit decommissioning activities. Additionally, de minimis quantities of vegetative material from basin closure may be placed in the landfill. The Permit to Operate shall expire October 26, 2021. 1. The Permit and Operations Plan are maintained on-site as required and were reviewed during the site inspection. 2. Pond 2 at Dan River has more vegetative matter than expected. Although this is a permitted waste stream, disposal at above de minimis quantities will result in the need to plan for potential landfill gas, more information on this issue is available in FID 1261168. 3. As of October 1, 2018, the facility is being operated by Trans Ash which is responsible for hauling materials to the landfill, daily operations of the landfill, and conducting other associated activities. Certified staff must be present when the landfill is in operation. Training records were documented and maintained on-site. Onsite operations training for incoming Trans Ash occurred on October 17, 2018. The following Trans Ash personnel are NC Certified Landfill Managers: Cecil Wilson, 17-02M-0285, expires 3/23/2020 Troy Medlin, 17-02M-002863, expires 3/23/2020 4. Ash disposal began on May 31, 2017. For the period between January 1, 2018 and November 27, 2018, the facility landfilled 776,336 tons. Ash was being placed in the landfill in Cell 3 at the time of the inspection. Cells 1, 2 and 3 are active and will be closed simultaneously upon final closure of the landfill. Additionally, ash continues to be transported by truck to Roanoke Cement for beneficial reuse. 5. The current goal is to landfill between 6,000 and 7,000 tons of coal combustion residuals per day. Above normal rainfall during October (8.4” of rain) and November (over 8.1”) slowed operations during those months. 6. Daily Observations Checklists were reviewed during the inspection and appear to be complete. 7. Dust records were reviewed during the inspection for October 1, 2018 through November 27, 2018. The records appeared complete and no concerns were noted. 8. Field density reports were reviewed during the inspection and appear complete; density tests which fail are retested. 9. Leachate and contact water generated by the facility is pumped from the landfill sumps to a leachate tank storage facility with a capacity of one million gallons. The leachate is sent via pipes to the City of Eden publicly owned treatment works (POTW) for treatment. The City has reissued the industrial users permit (IUP), Permit No. 1013-Discharge permit: Effective 3/1/2018, Expires 2/28/2019. The permit has been amended to increase the maximum quantity per day of leachate to 600,000 gallons. Leachate records were reviewed during the inspection. Leachate pumps have backup generators to continue pumping in the event of a power failure. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 3 of 4 10. Leachate system cleaning and flushing was completed between May 29 and 31, 2018. No issues were reported. Cell 3 – internal slope to Cell 2 on right 11. Edge-of-waste markers delineating the permitted disposal area were visible and being maintained. The markers are offset from the edge of the landfill liner. 12. All required signs have been posted. 13. Facility access roads are of all-weather construction and are well maintained. 14. Water trucks are utilized to control dust in the landfill and on roads. Water trucks were operating at the time of the inspection. Dust control checks occur daily at 9am and 2pm. Records were reviewed during the inspection and indicated that the facility has had numerous occasions requiring additional dust suppression during recent months. If dust cannot be controlled through other measures, operations at the landfill will stop until dust can be controlled. Conditions during the site inspection were cold, dry and breezy. Small puffs of dust were observed adjacent to the haul road inside the landfill. Observed dust did not leave the footprint of the landfill. Ensure that dust is being controlled on dry, breezy days. 15. Operational cover is being maintained as approved in the facility Operations Plan. Minimal soil is utilized for cover; landfilled materials are moistened and allowed to crust over to prevent windblown material. Additionally, Gorilla-Snot® can be used to aid in the process of crusting. 16. The semi-annual ground water, surface water and leachate monitoring samples were taken on November 1, 2017 and May 15-16, 2018. Side slopes and inactive areas have soil cover and have been reseeded in areas with inadequate vegetation. Side slopes were well vegetated and are being well maintained. Vegetative growth on side slopes and facility property are mowed regularly every four to six weeks or as needed. No significant erosion was observed.
