The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Met to discuss the permit modification for change in the permit holder from David Brantley & Sons to Wind
River Environmental. Received a permit submittal for this modification.
2. An authorization form for Wind River Environmental to discharge on this land application site
(currently permitted to David Brantley & Sons) is required for 2019 until the SLAS permit can be issued
to Wind River Environmental.
3. Field markers appear accurate. It appears that the approved setbacks are being met but were not measured at this
time. The site was GPS’s on the last visit and a new site map has been created – will be used for the new permit.
No sign of land application outside the permitted boundaries. No sign of heavy or recent application. No odors
noted.
4. The cropstand appears adequate – reseeding has been done on this site/could see the lines as it is becoming
established. Recent heavy rains have flooded/damaged an area on the low side of the site, Mr. Brantley
indicated that this area would be repaired. Continue efforts to improve cropstand & control weeds.
5. The required signs are posted and the access road is adequate.
6. Soil samples were taken and this office has received a copy of the soil report.
