The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Met to discuss the permit modification for change in the permit holder from David Brantley & Sons to Wind
River Environmental. Received a permit submittal for this modification.
2. Vegetation should be cut down around the facility for a distance of 5 feet around the tanks/facility to
provide the required distance as per Rule .0841(i)
3. The required signs are post and the access road is adequate.
4. The above ground tanks appear structurally sound.
5. It appears that septage is being transferred to/from the facility in a safe and sanitary manner. The area around
the dumpster & the sides of the dumpster are free of excess spills/fallen debris. No spills or leaks noted. No
vectors or odors.
6. However recent heavy rains did flood the compost area & dewatering box pit area. Noted an area to the side of
the dewatering box pit area that appeared to have septage waste on the ground surface – pointed this area out
to Mr. Brantley, clean up all septage waste in this area & dispose of it properly.
7. The approved setbacks are being met. No physical change in facility.
8. An authorization form for Wind River Environmental to discharge into this SDTF (currently permitted
to David Brantley & Sons) is required for 2019 until the SDTF permit can be issued to Wind River
Environmental.
