PART I: GENERAL FACILITY 1. This permit is issued by the North Carolina Department of Environmental Quality, Division of Waste Management (Division), Solid Waste Section (Section). In accordance with North Carolina Solid Waste Management Rule (Rule) 15A NCAC 13B .0201(d), a solid waste management facility permit shall have two parts: a Permit Approval to Construct (PTC) and a Permit Approval to Operate (PTO). The PTC must be implemented in accordance with Attachment 2 of this permit. The PTO must be
implemented in accordance with Attachment 3 of this permit.
2. The persons to whom this permit is issued (“permittee”) are the owners and operators of
the solid waste management facility.

5. By receiving waste at the facility, the permittee shall be considered to have accepted the
terms and conditions of this permit in accordance with Rule 15A NCAC 13B .0203(d).
6. Construction or operation of this solid waste management facility must be in accordance
with the Solid Waste Management Rules, 15A NCAC 13B, Article 9 of the Chapter 130A of the North Carolina General Statutes (N.C.G.S. 130A-290, et seq.), the conditions contained in this permit; and the approved plan. Should the approved plan and the Rules conflict, the Rules shall take precedence unless specifically addressed by a permit condition. Failure to comply may result in compliance action or permit revocation. 7. This permit is issued based on the documents submitted in support of the application for permitting the facility including those identified in the “List of Documents for the Approved Plan” which constitute the approved plan for the facility. Where discrepancies exist, the most recent submittals and the Conditions of Permit shall govern.

8. This permit may be transferred only with the approval of the Section, through the issuance of a new or substantially amended permit in accordance with applicable statutes and rules. In accordance with NCGS 130A-295.2(g), the permittee must notify the Section thirty (30) days prior to any significant change in the identity or business structure of either the owner or the operator, including but not limited to, a proposed transfer of ownership of the facility or a change in the parent company of the owner or operator of the facility. 9. The permittee is responsible for obtaining all permits and approvals necessary for the development of this project including, but not limited to, approval from appropriate agencies for a General or Individual National Pollutant Discharge Elimination System
(NPDES) Stormwater Discharge Permit – Permit No. NCG240007 and a sedimentation
and erosion control permit. Issuance of this permit does not remove the permittee’s
responsibilities for compliance with any other local, state or federal rule, regulation or
statute.

ATTACHMENT 3 CONDITIONS OF PERMIT APPROVAL TO OPERATE Part I: GENERAL FACILITY OPERATION CONDITIONS 1. The facility must be adequately secured by means of gates, chains, berms, fences, or other means approved by the Section to prevent unauthorized entry.

2. A sign shall be posted and maintained at the site entrance providing information on the types of acceptable wastes, permit number, and emergency phone number. 3. A responsible individual must be on-site during all operating hours of the facility at all times while open for public use. 4. Interior roadway must be of all-weather construction and maintained in good condition. 5. All sedimentation and erosion control activities must be conducted in accordance with the Sedimentation Control Act, N.C.G.S. 113A-50 et seq., and rules promulgated under
15A NCAC 4. All required sedimentation and erosion control measures must be
installed and operable to mitigate excessive on-site erosion and to prevent silt from
leaving the facility. The Section must be notified of any approved modifications to the
sedimentation and erosion control plan.
6. Facility construction, operations or practices must not cause or result in a discharge of
pollution, dredged material, and/or fill material into waters of the state in violation of the
requirements under Sections 401 and 4040 or the Clean Water Act, as amended.
7. Open burning of solid waste is prohibited. Fires must be reported to the Regional
Environmental Senior Specialist with 24 hours or the occurrence, followed by a written
notification within 15 calendar days of the occurrence.
8. Copies of this permit, the approved plans, and all records required to be maintained by the permittee must be maintained at the facility and made available to the Section upon request during normal business hours. 4110-COMPOST-, INGLESIDE COMPOST FACILITY, A LARGE TYPE 1 SOLID WASTE COMPOST FACILITY 1. The Permit Approval to Operate for the Large Type 1 Compost Facility shall expire
December 11, 2023. Pursuant to N.C.G.S 130A-294(a3)(2), the permittee must submit a request to the Section for permit review and must update pertinent facility plans. 2. The permittee must operate the Large Type 1 Compost Facility according to Rule 15A NCAC 13B .1406 and the approved plan (FID 1261072). 3. Per Rule 15A NCAC 13B .1406(6), the facility shall only accept the solid waste is permitted to received which are generated from the City of High Point and its incorporated service areas including portions of Guilford, Randolph, Davidson, and Forsyth Counties. The following wastes are approved to be used as the feedstocks for
composting and/or processing for mulch or engineering soil (FID 1261072). A permit
modification is required for a request to receive additional feedstocks.
a. Yard Waste as defined in Rule 15A NCAC 13B .0101(56) including land clearing
debris and yard trash as defined in N.C.G.S. 130A-294(a)(15) and (45),
respectively.
b. Garden waste, silvicultural waste, untreated, unpainted, and unglued wood waste
per Rule 15A NCAC 13B .1402(f)(1).
c. Municipal sludge incinerator ash that is generated from the East Side Wastewater
Treatment Plant in the City of High Point meets requirements set forth in Rule
15A NCAC 13B .1407 and testing criteria stated in the Operations Plan (FID
1261072) may be used as a component, up to 5% by volume or 5,000 cubic yards
at any time for producing engineered soil. The maximum on-site storage time of the municipal sludge incinerator ash shall not exceed (2) two months upon receiving. 4. At a minimum, the facility shall not receive the following waste for composting: a. Feedstocks that are in an anaerobic state. b. Coal combustion residuals as defined in N.C.G.S. 130A-290(a)(2b). c. Hazardous waste including household hazardous waste as defined in N.C.G.S. 130A-290(a)(8) & (8a).
d. Industrial solid waste as defined in N.C.G.S. 130A-290(a)(13b).

e. Municipal solid wastes as defined in N.C.G.S. 130A-290(a18a) excluding the wastes listed in the Permit Condition No. 3 of this part, Attachment 3 of this permit. f. Septage as defined in N.C.G.S. 130A-290(a)(32). g. Asbestos containing material or waste as defined in 40 CFR 61. h. Wastewater treatment sludge N.C.G.S. 130A-290(a)(34). i. Special wastes as defined in N.C.G.S. 130A-290(a)(40).
j. Any raw feedstocks that are known to produce products creating non-point source
pollution of waters of the State of North Carolina.
5. The permittee must conduct routine waste screening of the received wastes to ensure that
the prohibited, unauthorized, or the non-conformance wastes are properly separated from
the feedstocks. The removed wastes must be properly stored and disposed of at a
permitted facility according to the approved plan (FID 1261072).
6. The approved total capacity of the site is approximately 25,000 tons per year. A permit
modification is required for a request to increase capacity.
7. The NPDES permit (NCG240007) for managing and monitoring on-site stormwater or
wastewater at the facility must be implemented. Any leachate generated at the facility and/or any runoff from the facility must be managed in such a manner that ground water or surface water quality will not be adversely affected according to the approved Erosion and Sedimentation Control Plan (FID 1261072). The facility must be maintained to prevent the accumulation of stormwater and leachate on composting areas, storage areas, and roads.
8. In the event of a mechanical failure, the facility must immediately use its best effort to
utilize substitute equipment for completion of the composting operation.
9. The established odor management procedures must be followed to minimize odors at the
facility boundary per 15A NCAC 13B .1404(c)(4) and the approved plan (FID 1261072).
Upon receipt of an odor complaint, the facility operator must investigate and take action
as necessary to minimize the cause of the complaint. A copy of all written complaints
regarding this facility must be maintained for the duration of the permit including the
operator’s actions taken to resolve the complaints.

10. The facility must be operated in a manner that reduces the potential for vector attraction according to the approved Operations Plan (FID 1261072). 11. Windrow dimensions for active composting must be limited to approximately 6-feet high and 15-feet wide as described in the approved plan (FID 1261072) unless otherwise approved by a local Fire Marshal. A 25-foot minimum distance is required and constantly maintained between compost areas and swales or berms to allow for adequate access of firefighting equipment per Rule 15A NCAC 13B .1404(a)(8).
12. Per Rule 15A NCAC 13B .1406(10), compost process at the facility shall be maintained
at or above 131 degrees Fahrenheit for three (3) consecutive days and aerated to maintain
elevated temperatures.
13. All compost produced at the facility must meet the requirements of Rule 15A NCAC 13B
.1407 and the approved plan (FID 1261072).
14. Testing and reporting must be conducted in accordance with the requirements of Rule
15A NCAC 13B .1408 and the permit application. Compost process data must be
maintained in writing as required to document temperatures, moisture levels, and aeration
intervals. Bulk density and carbon and nitrogen ration (C:N) calculations must be
reviewed weekly.
15. Per Rules 15A NCAC 13B .1408(c) & (d), an annual report of facility activities for the fiscal year July 1 to June 30 must be submitted to the Section by August 1 of each year on forms provided by the Section. This report must include the amount of materials composted in tons. 16. The compost operation and the compost pad must be operated and maintained with sufficient dust control measures to minimize airborne emissions and to prevent dust from becoming a nuisance or safety hazard. a. Facility operation must meet Division of Air Quality Rules 15 NCAC 2D. Fugitive dust emissions from the facility that contribute to substantive complaints may subject the facility to the Division of Air Quality requirements as listed in 15A NCAC 2D .0540.
b. The composting piles or windrows shall not be turned on windy days (FID
1261072).

17. Groundwater quality at this facility is subject to the classification, monitoring, and remedial action provisions of 15A NCAC 2L. Groundwater monitoring may be required if there is an indication for the potential for groundwater contamination. PART VII: MISCELLANEOUS SOLID WASTE MANAGEMENT UNIT(S) Temporary Disaster Debris Staging Site (TDDSS), DS41-001 1. The TDDSS shall not receive disaster debris without the Section’s approval. When the site is activated following an emergency event: a. Only vegetative storm debris may be accepted in the TDDSS. Vegetative wastes typically include material similar to land clearing waste, yard waste, stumps, tree limbs, brush, and other naturally occurring vegetation. b. Activation must be requested and received from the Regional Environmental Senior Specialist for the facility. c. All permitted debris must be removed from the TDDSS within six (6) months from the activation date. d. Storm debris must be placed in rows with a maximum of 12 feet high and 25 feet
wide. A minimum of 15 feet row aisles must be maintained to separate piles.
