Pursuant to 15A NCAV 13B .0201(g), no later than 6 October 2019, the permittee must submit to the Division a permit amendment application prepared in accordance with 15A NCAC 13B .0535(b). PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection without records review/Pre-construction Facility Compliance Inspection of Phase II. STATUS OF PAST NOTED VIOLATIONS: None applicable OBSERVED VIOLATIONS: No violations were observed during this Facility Compliance Inspection; please see the ADDITIONAL COMMENTS section below. ADDITIONAL COMMENTS: 1) This Facility is a Construction and Demolition (C&D) waste Landfill and a Temporary Disaster Debris Staging Site (TDDSS). 2) This Facility is permitted to receive Construction and Demolition (C&D) waste as defined in NCGS 130A- FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section 5203_INSP_20181127 Page 2 of 2 290(a)(4) generated in Carteret, Craven, Duplin, Jones, and Onslow Counties. 3) This Facility is currently operating in Phase I Cell 2. 4) This Facility is currently receiving C&D waste from the Maysville C&D Recycling and Recovery Facility, permit #52-02 and Hurricane Florence generated Demolition waste from TDDSS’s activated in Carteret, Jones, and Onslow Counties. 5) Vegetative waste generated during Hurricane Florence is currently being disposed via an on-site Air Curtain Burner. 6) This Facility does not accept wastes containing asbestos. 7) The Facility is secured by means of a locked gate at the Facility entrance. The gate was open upon my arrival and Green Recycling Solutions Personnel were on-site during this Facility Compliance Inspection. 8) The Facility access road is of all-weather construction and is being well-maintained at this time. 9) Soil is stockpiled in an area adjacent to the landfill for use as weekly cover. 10) Permanent, highly visible, edge-of-liner markers delineating the waste disposal limits are maintained as required. 11) The waste type was reviewed; only Construction & Demolition waste was observed and no unpermitted waste streams were noted in the active disposal area during this Facility Compliance Inspection. 1) *Please ensure that all waste is being covered with a minimum of six inches of soil at least once per week or when the active disposal area reaches ½ acre in size, whichever occurs first. 2) Windblown debris is being well managed at this time. 3) Minimal windblown debris was observed on the landfill sideslopes and no windblown debris was observed leaving the Facility during this Facility Compliance Inspection. 12) Erosion was observed on the outside slopes in various areas. No off site erosion was observed. *Please repair all eroded areas by filling and grading at a slope not greater than 3:1. All repaired areas should be mulched and seeded to establish a groundcover suitable to prevent erosion of your land fill cap system. 13) Areas of sparse cover were oberved in several places around the landfill. *Please seed and mulch all areas of sparse cover to establish a cover crop suitable to prevent erosion of your landfill cap system. Please monitor the cover crop to ensure that it is established and maintained as required. Any areas where germination does not occur should be re-seeded and mulched promptly. 14) The leachate collection system and storage tanks were reviewed. The system appears to be functioning properly and is well maintained at this time. 15) Facility monitoring wells were reviewed. 16) All monitoring wells reviewed were properly labelled and secured with functioning locks. 17) *Corrective measures are required as a result of this Facility Compliance Inspection.
