STATUS OF PERMIT:
Active. The Permit to Operate issued on August 17, 2016 and shall expire March 31, 2020, according to Rule 15A
NCAC 13B .0201(g). Pursuant to Rule 15A NCAC 13B .0533(a)(2), no later than October 03, 2019, the permittee
must submit the Section a request for a permit review.
PURPOSE OF SITE VISIT:
Comprehensive Facility Compliance Inspection.

ADDITIONAL COMMENTS:
01 - The solid waste management facility Permit (DIN 9853) was registered in the Edgecombe County Register of
Deeds on April 07, 2010, Deed Book 1551, Pages 645 through 669 (DIN 10349).
02 - This permit approves the continued operation of the C&DLF Unit - Phases 1 through 4, on top of the closed unlined MSWLF, as well as the onsite environmental management protection facilities as described in the approved plans.
03 - This facility is permitted to receive permitted waste generated in the following North Carolina counties:
Edgecombe, Nash, Halifax, Wilson, Pitt, and Martin.
04 - The facility is approved to accept waste for disposal at an average rate of 13,000 tons per year. 05 - The scrap tires are being collected in an open top trailer for further processing.
06 - The facility has contingency plan for review posted on the peg board in the scale house. 07 - This Facility has 4 certified Landfill Operations Specialist: Travis Hudson: expires 10/03/20, Larry Moore: expires
05/24/19, Gloria A. Moseley, expires 10/21/20 and James Hucks: expires 05/24/19. 08 - The working face is a small area and appears to have good cover. Cover records are kept weekly and onsite. 09 - The Landfill is secured outside operational hours by means of locking gates and fencing.
10 - The signage at this facility has all required information posted. 11 - Random screenings are being done at least 2 times per week or more.
12 - Edge of Waste markers are present.
13 - The GW Monitoring wells are locked and have identification tags. 14 - Last date the GW Monitoring wells were sampled was July 23rd & 24th. 15 - Last date the Methane Gas Monitoring was done was October 18, 2018.
16 - Electronics are being stored under a shelter for further processing. 17 - Leachate issues noted during the last inspection have been repaired and looks good.
18 - This permitted facility has a Convenience Center located inside the landfill footprint.
19 - All waste is put into roll-off containers with signage indicating the waste to be deposited into the roll-off containers. 20 - Records were reviewed and were up to date.
21 - Overall management of this facility is good.
22 - Digital pictures taken during this inspection.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
