Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Met to discuss the permit modification for change in the permit holder from David Brantley & Sons to New Day Septic. Received a permit submittal for this modification. 2. Inspected the site & discussed inspection procedure, lime stabilization, grease dilution & current site map/setbacks. This site shall be kept as one field as per the current permit. 3. The required signs are posted, however one sign is slightly damaged. The access road is adequate. 4. The fescue crop on the top side of the site is not as well established as the lower side of the site, possibly due to the top side being land applied on more than the lower side – noted clover & vegetation other than fescue & thin areas. Reseeding has been done on this site, could see the lines as it is becoming established. Ensure that the entire area is utilized for land application, not just the top side of the field. Also ensure that adequate truck speed, truck pressure and an adequate spreader plate are used to achieve a wide thin spray path. 5. Field markers appear accurate. However vegetation needs to be removed from around some of the posts, particularly the corner one at the front of the site. This has been requested on previous audits. 6. It appears that the approved setbacks are being met but were not measured at this time. No sign of land application outside the permitted boundaries. No sign of recent or heavy application. No odors noted. 7. An authorization form for New Day Septic to discharge on this land application site (currently permitted to David Brantley & Sons) is required for 2019 until the SLAS permit can be issued to New Day Septic. 8. Soil samples were taken and this office has received a copy of the soil report.
