FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. Met to discuss the permit modification for change in the permit holder from David Brantley & Sons to New Day Septic. Received a permit submittal for this modification. 2. Inspected site & discussed inspection procedure. The required signs are posted, however one sign is slightly damaged. The access road is adequate. Two buried tanks. The approved setbacks are being met. The vegetation should be cut back for a distance of 5 feet as per Rule .0841(i). It appears that septage is being transferred to/from the facility in a safe and sanitary manner. No leaks or spills noted. 3. The installation of a concrete screening box is planned. Requested a written explanation of this screening box and how the septage debris shall be contained & disposed of properly – to be added to the submitted application for this permit modification. 4. An authorization form for New Day Septic to discharge into this SDTF (currently permitted to David Brantley & Sons) is required for 2019 until the SDTF permit can be issued to New Day Septic.
