It appears that the septage spills noted on the last audit of 4/06 were cleaned up.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Met to discuss the permit modification for change in the permit holder from David Brantley & Sons to New
Day Septic. Received a permit submittal for this modification.
2. Inspected site & discussed inspection procedure. The required signs are posted, however one sign is slightly
damaged. The access road is adequate. Two buried tanks. The approved setbacks are being met. The
vegetation should be cut back for a distance of 5 feet as per Rule .0841(i). It appears that septage is being
transferred to/from the facility in a safe and sanitary manner. No leaks or spills noted.
3. The installation of a concrete screening box is planned. Requested a written explanation of this screening
box and how the septage debris shall be contained & disposed of properly – to be added to the submitted
application for this permit modification.
4. An authorization form for New Day Septic to discharge into this SDTF (currently permitted to David
Brantley & Sons) is required for 2019 until the SDTF permit can be issued to New Day Septic.
