OBSERVED VIOLATIONS:
Rule .0844(b) All permitted septage management firms shall display decals or lettering on each side of every pumper
vehicle operated by the firm. The decals or lettering shall include the name, town name, phone number, and septage
management firm permit number. All decals or lettering on the pumper vehicle shall be no less than three inches in height
and plainly visible. The number “7” in the permit number on the vehicles inspected at this time (tag#AS3887 &
JA8419) was less than three inches in height. Instructed Mr. Hurley to replace the number “7” with new decals of
at least three inches in height. Send photos of the corrected lettering to connie.wylie@ncdenr.gov.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected a new vehicle, tag#JA8419, VIN 54DC4W1B4JS802413, 950 gallons. Tank appeared structurally
sound & securely attached. Outlet covers were on and no drips/leaks were noted.
2. Inspected a previously permitted vehicle that had had the driver side door repainted/relettered. Tag#AS3887,
noted discrepancies in the VIN number as listed on the last application – ensure that the correct VIN number is
listed on the next firm application. Tank appeared structurally sound & securely attached. Outlet covers were on
and no drips/leaks were noted.
3. Truck logs were reviewed at this time. It appears that the required information is being maintained.
