The Permit was issued March 20, 2014 and will expire on March 05, 2019. Submit application for new permit at
least six months prior to expiration.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
01 - The Permit to Operate for this facility was issued on March 20, 2014 and is set to expire on May 05, 2019. Permit
renewal application in process. 02 - This facility has 5 Certified Transfer Station Operations Specialist: Gilbert W. Stephen, expires 11/5/18, Stanley Lassiter, expires 11/5/21, Judy M. Baker, expires 5/10/19, Donnell Cooper, expires 11/16/19 and Michael Bradly,
expires 11/5/21. 03 - Facility has signage with required information. 04 - Facility has fencing with a locking slide gate at the entrance. Repairs be done to gate during this inspection.
05 - The facility road is constructed of ALL Weather materials. 06 - Facility is closed on ALL Sundays and the closed Holidays are; New Year’s Day, Martin Luther Day, Good Friday, Memorial Day, Independence Day, Labor Day, Veterans Day, Thanksgiving Day and the day after, Christmas Eve, Day
and the day after.
07 - Facility operating hours are, Monday – Friday 8:30 am to 5:00pm. Saturday hours are, 9:00am to 12:00pm.
08 - It was noted on the last inspection (10/10/18) that the area around the compactor on the tipping floor could use a little more attention on cleaning. This area was clean as was the rest of the tipping floor.
09 - There is a convenience center located inside of this Transfer Station footprint that was keep very well. 10 - Electronics were being prepared for removal. 11 - Minimal windblown trash was noted during this inspection that was conducted at mid-day.
12 - Facility also has scrap shingles for recycling. Area was maintained well. 13 - Scrap tires are laced into a closed top trailer. 14 - Overall management of this facility was good.
