PURPOSE OF SITE VISIT: Comprehensive Inspection STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: None. The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. The facility is permitted to receive, consolidate and transfer yard trash (as defined in 15A NCAC 13B .0101(55)) for storage and removal. Typically, the site receives and manages leaves collected curbside by the City of Winston-Salem Sanitation Division. 2. Operations at the site are managed by Wallace Farms, Inc., a contractor that also manages the city’s Overdale Road Yard Waste Facility (permit # 34-09) and Forum 52 Large Type 1 Compost Facility (permit # 34-23). 3. City trucks unload leaves at the site and push them into piles. Wallace Farms staff then manage the leaf stockpiles during their temporary storage at the site, and load them into trucks, transporting them as needed. 4. Mr. Lentz stated that the majority of the leaves are transported directly to the Davie County Compost Facility (permit # 30-04), which is owned and operated by Wallace Farms. He further explained that some of the leaves are transported to other city sites so that they may be made available to residents as compost in the spring of the year. The city now contracts with Wallace Farms to manage operations at all of its yard waste/compost sites. 5. Mr. Lentz stated that leaf loads had begun arriving at the site the previous week. He added that, so far this season, 18 loads of leaves had been transported from the site. Using Mr. Lentz’s formula of 90 to 100 cubic yards per truck load, this equates to an estimated total of 1700 cubic yards. 6. The site was observed to be clean and well-maintained. 7. The facility is secured by a perimeter chain link fence with two locking gates to prevent unauthorized entry. Gates are locked except when facility trucks are unloading. 8. Proper signage was observed at the entry gate of the facility. 9. The site is slopes to direct surface water to the city’s storm drains. 10. The entire site is asphalt paved. A truck ramp had been staged near the center of the site, and Mr. Lentz pointed out that mulch is used to cover the metal ramp to enable loaders to operate on it safely. 11. A small area behind the truck ramp was observed to be holding water. This is the area that city staff fill with gravel each season to mitigate the degrading asphalt. Following the inspection, in a telephone conversation, Assistant Sanitation Director Nicholas Geis stated that he would send a crew with a load of gravel to repair the low area before close of business. 12. City crews have painted a dashed white line along the rear (north side) of the asphalt paving. The line marks the required 50’ buffer between the property line and facility operations, and is visible over the entire length of the paved operations area. 13. A pile of mulch was observed near the northwest corner of the site. Mr. Lentz stated that the mulch is used to refresh the truck ramp. A portion of the mulch pile had exceeded the buffer line, and Mr. Lentz used the loader to move it out of the buffer and into the operations area during the inspection. 14. Leaf piles at the site appeared to meet the maximum size of 15’ high and 25’ wide, as specified in the Permit to Operate. 15. The site was found to be clean and well-managed, with ample space for trucks and equipment to maneuver safely. All areas were easily accessible for firefighting equipment should the need arise. 16. Mr. Lentz indicated that he and his staff monitor the leaf piles and ensure that they are turned frequently to prevent elevated temperatures. 17. Please ensure that an individual with operational responsibilities is onsite during all operations at the facility, as required by the Permit to Operate.
