OBSERVED VIOLATIONS:
1. .0844(b) all permitted septage management firms shall display decals or lettering on each side of every pumper
vehicle operated by the firm. The decals or lettering shall include the name, town name, phone number and permit
number. All decals or lettering on the pumper vehicle shall be no less than three inches in height and plainly
visible. Inspected the new vehicle in regards to issuing the first firm permit. The town name (Lumberton) was
displayed on each door and was less than three inches in height. Correct the town name on both sides of
this vehicle at three inches or greater in height. Contact this office to schedule another inspection.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The firm name, phone number & permit number were correctly displayed on both sides of the vehicle.
2. Outlet covers were on and no drips/leaks were noted.
3. The tank appears structurally sound & securely attached.
4. The firm permit cannot be issued until the vehicle is brought into compliance and another inspection is
performed. No septage pump jobs can be performed until after the firm permit is issued.
