Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. A notification letter was sent on September 4th stating that the renewal submittal was required 90 days prior to the expiration date. Was informed that the renewal application was mailed but as of this date (less than 60 days from the expiration date) this office has not received the renewal application. Also informed that the soil samples have been taken, have not yet received the soil analysis. 2. Appears the understory control was performed on Field 2 since the last inspection. Continue efforts to control excess vegetation. Field 1 is inactive due to past timbering. 3. Severe pot holes/ruts were noted on the driving lanes which need to be repaired. 4. No sign of land application outside the permitted boundaries. It appears that the approved setbacks are being met but were not measured at this time. Field markers appear adequate. 5. Site is in use. Slight odor noted. Ensure that pH 12 is present in all waste at time of application. 6. Heavy applications noted in some areas.
