Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. The fescue on Field 2 appears to be suffering. Mr. Oaks stated that he would look into improving the cropstand, possibly replanting. Future plans are to start using Field 1 again (this field is currently inactive). The fescue crop will have to be reestablished on Field 1 – Mr. Oaks is planning the replanting of this field. This permit expires in June 2019 and Field 1 can be included back into the new permit in regards to “gallons”. 2. The field markers are missing at the swale on Field 2. The markers have to be reposted before land application may occur. This was also previously noted on the audit report of 7/30/2018. 3. The required signs are posted and Mr. Oaks recently repaired the access road. 4. No sign of land application outside the permitted boundaries. It appears the approved setbacks are being met, but were not measured at this time. No sign of recent or heavy application. No odors noted. 5. Reviewed the required information to be maintained for the site logs. Ensure that each separate discharge event is listed on the site (should not be a daily total). 6. Mr. Oaks stated that the required soil samples have been taken. Soil analysis has not yet been received.
