PURPOSE OF SITE VISIT:

STATUS OF PAST NOTED VIOLATIONS:
.0838(a)(15 & 18) No new grease burns were noted on the fields during this visit. The fields were harvested. A copy of
the site logs and a letter from Emily Cooper were received by this office. The Notice of Violation issued August 20,
2018 is resolved.
OBSERVED VIOLATIONS:
1. .0838(a)(2) Access roads or paths crossing or leading to the disposal area shall be posted “No Trespassing” and a
legible sign stating “Septage Land Application Site” shall be maintained at each entrance to the land application
area. The “Septage Land Application Site” sign is damaged - repost a new sign.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Remove the hay bales from the perimeter of the site particularly the ones sitting along the access path on the
south side of the site.
2. At this time it appears that rye is coming up on the Field 1, but not on Field 2. For land application to occur this
winter an adequate cover crop shall be established uniformly over each field.
3. Field markers appear accurate, however a post is down at the front corner of Field 1 (near the trailer) –
setback at this location is 130 feet from the center line of road & 500 feet from the trailer – repost this
marker.
4. No sign of land application outside the permitted boundaries. It appears that the approved setbacks are being
met but were not measured at this time.
5. No recent application and no odors noted.
6. The access road is adequate.
7. The submitted site logs are under review. This site is managed as two fields.
8. Soil samples shall be taken by permit holder & submitted for analysis.
