or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected a replacement vehicle to be added to the firm permit (new chassis/old tank repainted).
Tag#YA135059, VIN 1HTMMAAN09H068841, 2375 gallons. The lettering is correct on both sides of this
vehicle. The tank appeared structurally sound & securely attached. The outlet covers were on and no spills or
leaks noted.
2. Also inspected the second vehicle covered under the firm permit – tag# YA134436. The tank appears
structurally sound & securely attached. The lettering is correct on both sides of this vehicle. The outlet covers
were on and no spills or leaks noted.
3. Truck logs were not reviewed at this time.
4. Discussed the firm permit renewal to be submitted by the end of December. Mr. Washington stated that he plans
to attend the Raleigh training class in December.
