The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Land application occurred on Field 2 during this visit. PH was checked & based on the color appeared to be
barely a 12 (or slightly less). Discussed with Frankie Jones that application methods need to be reviewed
with drivers – was pH checked on site immediately prior to discharge, was pH checked earlier and held
at pH 12 for a half hour for domestic & did adequate mixing occur. No land application shall occur
unless pH is 12 or higher. No significant odor noted.
2. Noted scattered areas of thin or bare patches of groundcover which may be from heavy application. Also noted
matted areas which result from heavy application. Discussed with Mr. Jones that matted areas should be
raked loose to prevent crop damage and that application methods should involve adequate truck speed,
pressure and a spreader attachment to spray in a thin wide path. Continue efforts to improve the cropstand
& control weeds per the advice of Georgia Love, regional agronomist.
3. Was informed that rye has been seeded on the fields but is not evident at this time. No land application shall
occur this winter unless an adequate winter cover crop is established.
4. Hay bales are currently located along the perimeter of field 2 and should be removed in a timely fashion.
5. The required signs are posted and the access road is adequate.
6. Field markers are adequate and it appears that land application is occurring within the permitted boundaries.
The current approved setbacks are being met.
7. Reviewed new areas (pre-operational inspection) to be added to the permitted site – required setbacks were
measured, seasonal water table was looked at and cropstand (at this time) was evaluated. Orange flags were
posted along new field perimeters and also a new field division between the two fields. These orange flags will
need to be replaced with permanent markers. The site was GPS’d for a new site map & accurate acreage for
the new permit. The new areas cannot be used for land application until the new permit is issued. Minor
corrections were made to the SLAS application during this meeting. The permit submittal is currently under
review.
8. Soil samples were taken by the permit holder. Obtained a copy of the soil report.
9. Two storage tanks have been located on site. Appear structurally sound and the current location is in accordance
with setback requirements. Coordinates were taken for the tank location & this office has received an
application for a SDTF permit.
