Hi Pieter:

Below are the comments on the CQA Rpt (FID125657) for Area 3, Cell 1.

FID 1256891
1. (Section 3.6) There is no Appendix O in the report. Please correct this typo.

2. (Appendix G, Soil Liner Data) The value of the in-place dry density of sample DC-11 stated in Soil
Liner Record Test Summary General Fill (on page 322) is inconsistent with the lab data report (on page
352) dated 06/11 through 06/16/2018.

3. (Appendix H) According to the Roll#/Lo# of the GCL used for lab testing of interface friction angle, this
GCL material is not collected from the GCL shipped for this project. Please explain why the testing
results are acceptable.

4. (Section 7.0 & Appendix I) Please address the following concerns related to installation of GCL at this
project.
a. Provide the document/statement that the surface receiving GCL has been properly prepared per Rule
15A NCAC 13B .1624(b)(9)(c).

b. State or verify that geotextile of the installed GCL has been inspected for assurance of a broken
needle(s) is not present per Technical Specification Section 02776, Part B.2.

c. Provide post-installation documents as specified in the Technical Specification Section 02776, Part
C.2: certificates and warranties.

d. Please provide manufacturer MQA/MQC test result (Certificate of Analysis 2018) on GCL Cap Roll
# 111415703 & Base Roll # SN01015169, which are not available in the CQA report (pp. 525
through 527).

5. (Appendix J)
a. The test on seam connecting Panels P/P56 is not available in the Non-Destructive Test Log.
b. Provide the manufacture warranty for installed geomembrane.

6. (Section 9.3 & Appendix K) Please provide test results of carbonate content (by ASTM D 3042) for both
No. 78 & No 57 stones (per Technical Specification Section 02710 & Table 6.1 of the CQA Plan). As
mentioned in Section 9.3 the previous test results are acceptable as long as the material coming from the
same sources.

7. (Appendix L) Provide a manufacture “Needle-Free” statement for the specified and installed geotextile
material which intimately contacts geomembrane liner.

8. (Appendix N)
a. (Clay Layer As-Built) Two survey points – 852D & 853D are not shown on the drawing.

b. (Protective Cover As-Built)
i. The following survey points are not shown the drawing: 559F, 653F & 858F.

ii. It will be helpful to show the boundaries of the protective layers/leachate collection systems
constructed with different components as described in Section 2.1 of the CQA Report.

c. (Drainage Aggregate As-Built)
i. The following IDs of survey points on the drawing are inconsistent with those in the table.
