Figure 1 illustrates the location of this facility on the Wilson Quadrangle U.S.G.S. Topo Map. This facility contains one permanent structure which is used as a gasoline convenience store. The site formerly contained (2) 6,000 gallon gasoline, (1) 4,000 gallon gasoline, & (1) 4,000 gallon diesel USTs. All of the gasoline USTs are still in use at this facility. The 4,000 gallon diesel UST, which was last used at this facility in November, 2015, was removed on December 17, 2015. Figure 2 illustrates the overall layout of this site and the UST locations. Table 1 summarizes the UST history for the Friendly Mart site. A release from the UST system was suspected on 12/17/15 based on elevated PID readings observed in soil samples collected from around the UST and the presence of a floating layer of diesel in the UST excavation. Groundwater was observed at a depth of approximately 4’ to 5’ below surface in the UST pit during soil abatement activities. The site and adjacent properties are used for commercial, residential and industrial purposes. Municipal water supplies are available to properties within 1,500’; however, adjacent properties are served by a local water supply well. 2.0 – UST CLOSURE REPORT I. General Information A. Facility/UST Status: The UST removed from this facility was last in use in November, 2015. The facility in still in operation for commercial petroleum sales. 3. Surrounding Property Use: Industrial/Residential/Commercial 4. Site Geology: Native soils consist of silt with varying amounts of clay and sand. Bedrock was not encountered. Groundwater was observed in the remedial excavation at a depth of approximately 4’ to 5’ below surface, and in remedial excavation monitoring well MW-1 at a level of 4.57’ below top of casing. A geologic log is contained in Appendix C. Other pertinent site information is contained on the GW/UST-2 Site Investigation Form which is included in Appendix A. II. Closure Procedures A. Notification and Tank Preparation Page - 3 NCDEQ: Henry Nemargut Engineering Services notified the UST Section of the NCDEQ Raleigh Regional Office of the intent to permanently close by submitting a Notice of Intent: A copy of UST-3 is included in Appendix B. B. Residuals Petroleum liquids were removed from the UST by Rose Oil Company, Inc. and transferred to other vessels owned by Rose Oil Company, Inc. Liquids disposal manifests have not been made available to Henry Nemargut Engineering Services. C. Excavation The UST closure project at the Friendly Mart facility was conducted on December 17, 2015. Ready Contractors excavated the product and vent pipes, drained and vacuumed them to avoid release of product into the surrounding soils, then removed the lines from the tank. The UST was then exhumed from the ground on December 17, 2015. Figure 2 illustrates the layout of the project site and the locations of the tanks removed during this closure project. Henry Nemargut Engineering’s staff inspected the tank for structural integrity upon removal. Documentation concerning the conditions the tanks is contained in Section 3.0 of this report. Following removal of the UST, the excavation was visually inspected for the presence of free product and groundwater. Water was observed in the tank pit at a depth of approximately 4’ to 5’ below surface. Free product was evident on the pit groundwater during the removal project. Petroleum odors were noted around the UST and in overburden soils. The dimensions of the excavation created by the removal of the UST were approximately 36' long by 17' wide by 13’-14’ deep. D. Contaminated Soil Contaminated soils were noted during abatement activities conducted in December, 2015. Approximately 409.86 tons of contaminated soil generated during UST closure operations and remedial excavation activities were transported and disposed of at Soilworks, Inc. in Selma, NC. Composite samples obtained from these soils indicated TPH-GRO/DRO contaminant levels in excess of NCDEQ Action Levels Appendix D contains a soil disposal certificate for the soils disposed of from this site during Initial Abatement Action activities. III. Site Investigation Site investigation activities are described in Section 3.0 through Section 5.0 of this report. Page - 4 3.0 - INITIAL RESPONSE AND ABATEMENT ACTIVITIES Initial abatement activities were conducted at the Friendly Mart on December 17-21, 2015 by removal and disposal of approximately 409.86 tons of contaminated soil removed from the remedial excavation. Ready Contractors excavated the fill, product and vent pipes, cut and drained them to avoid release of product into the surrounding soils, then removed them from the tank. The UST was then exhumed from the ground. The tank was inspected for structural integrity upon removal. The UST was in poor condition, with at least 2 holes noted in its shell. After removal, the UST was transported to Foss Recycling in Wilson, NC for recycling. A scrap metal receipt for the UST removed during abatement is contained in Appendix F. Appendix C contains a soil disposal certificate for the soils disposed of from this site during Initial Abatement Action activities. Figure 4 illustrates the approximate limits of excavation for soils removed from the project site. Following removal of the UST, the excavation was visually inspected for the presence of odors, free product and groundwater. Water was observed in the tank pit at the time of removal. Free product was evident floating on the pit groundwater during the removal project. Petroleum odors were noted around the UST. Based on the observed free product and elevated PID readings noted around the USTs, a decision was made to excavate, transport and dispose of off-site suspected contaminated soils encountered during UST closure. Following removal of the UST, additional excavation was conducted at the site on December 17, 2015 in areas of the site that exhibited elevated PID readings and free product. Excavation was ceased after all suspected contaminated soils were removed from the UST area. Appendix D contains soil disposal manifests for the soils disposed of from this site during Initial Abatement Action activities. 4.0 - SOURCE INVESTIGATION On December 17, 2015, following the excavation of the UST and soils in the tank area, soil samples were collected in accordance with NCDEQ guidelines. Soil samples were obtained from the excavation walls since the UST pit contained water. The soil samples were collected from a depth of approximately 3’-4’ from the perimeter of the UST excavation based on the observed depth to water in the UST pit. Soil Samples were analyzed via TPH-GRO/DRO and/or EPA 8260/8270/MADEP EPH/VPH in areas were over-excavation was conducted. (2) Stockpile soil samples were obtained from stockpiled soils for TPH analyses from soils excavated during UST closure and remedial operations. TPH laboratory analytical results are summarized in Table 2. Risk-based results are tabulated in Table 3, with TPH/VPH/EPH results depicted on Figure 3. A geologic log of excavation for these activities is contained as Appendix C. The soil samples collected at 3055 Stantonsburg Road were labeled with sample location, depth, and date collected. All soil samples were submitted to Pace Analytical, Inc for analyses. The soil samples were obtained as specified in the collection, preservation, and handling section of the specific method. Each container was labeled with sample location, analyses to be performed, time, date, and the sampler's name. They were then placed in a cooler and chilled with ice to approximately 4oC in preparation for transportation to the analytical laboratory utilizing EPA approved chain of custody procedures. Page - 5 Results from the release confirmation soil samples were above current NCDEQ action levels for TPH-DRO/GRO compounds for stockpile soil samples. Laboratory TPH analytical results confirm that contamination was present in the soils disposed of from the project site, with TPH-DRO compounds noted at levels of 179 mg/kg to 1,480 mg/kg and TPH-GRO levels noted at levels of 14.6 mg/kg to 86.5 mg/kg in composite stockpile soil samples X-1 & X-2 collected during abatement activities. Soil samples were collected from the perimeter of the UST excavation where over-excavation was conducted to obtain risk-based analytical data for the site. Results from the risk-based and TPH soil samples were below NCDEQ lowest MSCCs or Action Levels for all 8260/8270/VPH/EPH/TPH compounds for pit wall samples. Appendix F contains copies of the laboratory analytical reports and the chain of custody records for the soil samples collected at the project site. 5.0 – GROUNDWATER INVESTIGATION 5.1 Monitoring Well Installation A total of one North Carolina Type II groundwater monitoring well was installed on-site during initial abatement action activities per discussions with Brenda Hafshejani of the UST section. Figure 4 illustrates the location of this monitoring well, denoted as MW-1. MW-1 was placed in the diesel UST pit which was excavated to below the water table during remedial excavation activities. The monitoring well was constructed of 2-inch Schedule 40 PVC pipe with 3.75’ of casing and 10 feet of 0.010 inch slotted screen. Based on the assumption that the contaminants being addressed were primarily hydrocarbon constituents with specific gravities of less than 1.0, the Type II monitoring well was installed so that the screened interval intersected the shallow groundwater table at the time of installation. 5.2 Monitor Well Sampling and Analyses On June 5, 2015, monitor well MW-1 was sampled for analyses utilizing EPA Methods 6200B plus MTBE, IPE, & EDB, 625 + 10 peaks, Pb, and MADEP Methods for VPH/EPH as appropriate based on UST contents. Before sampling, the monitor well was purged with a submersible pump by removing 3 well volumes to ensure that the samples were representative of the actual groundwater conditions. The monitor wells were then sampled using the submersible pump at a reduced pumping rate. Groundwater was detected in monitor well MW-1 at a depth of approximately 4.57’ below top of casing prior to purging and sampling. All samples were placed in laboratory supplied glassware, labeled with sample location, analysis to be performed, time, date and sampler's name. The sample jars were then immediately placed in a cooler, chilled with ice to approximately 4oC in preparation of transportation to an analytical laboratory utilizing EPA approved chain-of-custody procedures. Page - 6 Laboratory analytical results from MW-1 indicate the presence of 4 compounds/compound groups above 15 NCAC 2L Standards. Benzene was noted at a concentration of 37.9 ug/l in MW-1, above its 2L Standard of 1 ug/l. Naphthalene was noted at a concentration of 21.8 ug/l in MW-1, above its 2L Standard of 6 ug/l. No compounds were detected in MW-1 at levels above GCLs. Table 4 summarizes the laboratory analytical results for the groundwater samples. Appendix D contains a copy of the laboratory analytical report and chain of custody record for the groundwater samples. 6.0 - CONCLUSIONS AND RECOMMENDATIONS 6.1 General Summary The activities conducted for this Initial Abatement Action Report at the Friendly Mart have been completed. From a review of all information gathered during this project, Henry Nemargut Engineering Services makes the following conclusions: o (1) 4,000 gallon diesel UST was removed from 3055 Stantonsburg Road on December 17, 2015. o A release was confirmed from the UST in the form of contaminated soil and free product floating on UST pit water. Corrosion holes were noted in the shell of the UST. o Approximately 409.86 tons of contaminated soil generated during UST closure operations and remedial excavation activities were transported and disposed of at Soilworks, Inc. in Selma, NC. Composite samples obtained from these soils indicated TPH-DRO compounds noted at levels of 179 mg/kg to 1,480 mg/kg and TPH-GRO levels noted at levels of 14.6 mg/kg to 86.5 mg/kg. o Laboratory analytical results for the risk-based and TPH soil samples obtained on December 17, 2015 following completion of remedial excavation indicate that soil contamination in the vicinity of the diesel UST has been remediated to below NCDEQ lowest Maximum Soil Contaminant Concentrations (MSCCs) for all 8260/8270/VPH/EPH/TPH compounds. o One groundwater monitoring well, MW-1, was installed in the UST remedial excavation since groundwater was encountered during remediation in this pit. MW-1 was placed in the portion of this excavated pit which exhibited free product. Groundwater was observed at a depth of approximately 4.57’ below top of casing in this well. Free product was not observed this monitoring well during sampling on January 28, 2016. o Laboratory analytical results from MW-1 indicate the presence of 4 compounds/compound groups above 15 NCAC 2L Standards. Benzene was noted at a concentration of 37.9 ug/l in MW-1, above its 2L Standard of 1 ug/l. Naphthalene was noted at a concentration of 21.8 ug/l in MW-1, above its 2L Standard of 6 ug/l. No compounds were detected in MW-1 at levels above GCLs. The evaluation was conducted based on the scope of work and level of effort desired by the client and with resources adequate only for the scope of work. Our findings have been developed in accordance with generally accepted standards for Initial Abatement Action Reports in the State of North Carolina, available information and our professional judgment. No other warranty is expressed or implied. The data presented in this report are indicative of conditions at the precise locations sampled and the time the sample was collected. Additionally, the data obtained from the samples would be interpreted as meaningful with respect to the parameters in the laboratory reports. Permanent closure -For permanent closure, complete all sections of this form. Change-in-service -For change-in-service where UST systems will be converted from containing a regulated substance to storing a non-regulated substance, complete sections I, II, III, IV, and VIII Effective February 1, 1995, all UST closure/change-in-service reports must be submitted in the format provided in the UST-12 form. UST closure and change-in-services must be completed in accordance with the latest version of the Guidelines for Tank Closure.
