PURPOSE OF SITE VISIT:
Partial Inspection
STATUS OF PAST NOTED VIOLATIONS:
1. RESOLVED: 15A NCAC 02C .0108(k) states: “All non-water supply wells, including temporary wells, shall be
secured with a locking well cap to ensure against unauthorized access and use.” In a letter from Smith Gardner,
Inc., dated August 30, 2018, Joan Smyth certified that all landfill gas and groundwater monitoring wells at
the Phase 1 and 2 landfills and the C&D landfill had been upgraded or repaired to ensure that each had
been secured with a locking well cap.
2. RESOLVED: 15A NCAC 02C .0108(l) states: “All non-water supply wells shall be equipped with a steel outer
well casing or flush-mount cover, set in concrete, and other measures sufficient to protect the well from damage
by normal site activities.” In a letter from Smith Gardner, Inc., August 30, 2018, Joan Smyth certified that
all landfill gas and groundwater monitoring wells at the Phase 1 and 2 landfills and the C&D landfill had
been upgraded or repaired to ensure that each has been enclosed in a steel casing and set in a concrete pad.

3. RESOLVED: 15A NCAC 02C .0108(o) states: “Each non-water supply well shall have permanently affixed an
identification plate. The identification plate shall be constructed of a durable, waterproof, rustproof metal or other
material approved by the Department as equivalent and shall contain the following information:
(1) well contractor name and certification number;
(2) date well completed;
(3) total depth of well;
(4) a warning that the well is not for water supply and that the groundwater may contain hazardous materials;
(5) depth(s) to the top(s) and bottom(s) of the screen(s); and,
(6) the well identification number or name assigned by the well owner.”
In a letter from Smith Gardner, Inc., dated August 30, 2018, Joan Smyth certified that all landfill gas and
groundwater monitoring wells at the Phase 1 and 2 landfills and the C&D landfill had been upgraded or
repaired to ensure that the required permanent identification plate has been affixed to each.
OBSERVED VIOLATIONS:
No new violations were observed.
ADDITIONAL COMMENTS
1. GWM-9A, the flush-mount groundwater monitoring well located adjacent to the access roadway on the north
side of the active landfill, was inspected. During previous inspections, this well could not be located. Mr.
Brushwood stated that staff had located the well using a metal detector. The well was observed to be in good
repair. County staff had mown the area surrounding the well, and had also marked the location with orange
cones to ensure that it would not be damaged during mowing or maintenance operations.
2. The slopes of the active landfill were observed from the perimeter access roadway and found to be in good
condition. One small area of the southern slope had erosion rills from the heavy rains that resulted from
Hurricane Florence. Mr. Brushwood stated that this was the last area requiring repair from the storm. He
explained that staff had been proactive ahead of the storm, and that this had been helpful in limiting the damage
to the slopes.
3. Access roadways were found to be in good condition, with only a few small areas where ruts had developed.
Mr. Brushwood stated that, again, staff had been working proactively to keep up with the repairs necessitated by
the storm water damage and the heavy truck traffic going in and out of the facility.
4. Windblown litter was also observed to be well-controlled. A small amount of trash was observed just below the
working face, and Mr. Brushwood stated that the temporary worker assigned to do litter pick-up would be
taking care of that before the end of the day.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
