Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 AREAS OF CONCERN AND COMMENTS: TRUCK LICENSE PLATE # VIN # TANK CAPACITY (gal) YA143056 2NKMHD7X55M075130 1000 septage 750 fresh water 1) All lettering indicating firm name, town name, phone number, and septage management firm permit number were permanently attached to each side of the truck and plainly visible. All lettering measured greater than 3 inches in height. 2) The tank was constructed of metal and appeared to be structurally sound. 3) The tank was bolted to the truck frame. 4) The septage drain valve appeared to be in working order. 5) No septage leaks were observed. 6) All port covers and caps where in place and appeared to be in proper working order. 7) Truck logs were not required for new truck inspection.
