STATUS OF PERMIT:
Active – Issued April 21, 2014.This permit shall expire April 21, 2019. Pursuant to 15A NCAC 13B .0201(g), no later
than October 21, 2018, the owner or operator must submit a request to the Section for permit review and must update
pertinent facility plans including, but not limited to, the facility operation and waste screening plans.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS:
01 – This facility inspection was conducted to observe operations after Hurricane Florence. 02 – Extra waste was being hauled to this facility due to no access to other MSWLF’s due to Hurricane Florence.
03 – This facility has kept up with the extra waste coming in, 04 – Wind blown waste was at a minimum during this inspection. Staff picking up during this inspection.
05 – Facility is using SMC (posi-shell) and soil daily cover. Soil cover stockpiled on working face.
06 – Several small leachate breakouts were observed on-site. One in cell#7 and one in cell#12. Repairs started. 07 – PTO and Operations plan reviewed. 08 – Cover logs reviewed and up to date.
09 – Waste screening records reviewed. 10 – Working face is kept to a small area.
11 – Matting and seeding on bare areas is being done during this inspection.
12 – Leachate storage has about 40% left in tanks. 13 – One leachate tank has small leak. Leachate is contained.
14 – Overall site looks good.
15 – Digital pictures taken during this inspection.
