PURPOSE OF SITE VISIT:
Follow up to NOV issued August 1, 2018
STATUS OF PAST NOTED VIOLATIONS:
.0832(c)(1 & 2) No recent application to check pH at this time. Enviro Choice provided a written certification
regarding the violation & has implemented procedural changes to ensure a pH of 12 or higher is present for land
application to occur. Enviro Choice has also implemented additional training of field staff.
The Notice of Violation issued August 1, 2018 for permit #SLAS 78-22 is resolved.
OBSERVED VIOLATIONS:
1. .0838(a)(12) All septage discharges, including aerial drift from discharges, shall be made within the permitted
boundaries of the land application site. Permit Condition 12 Any discharge of septage outside the permitted
boundaries via runoff, aerial drift etc is prohibited.
Observed (with Cody Hunt) a discharge path outside the permitted boundary at the corner post on the south west
corner of Field 5.

Ensure that all septage discharges shall be made within the permitted boundaries of the land application
site.
2. .0838(a)(15) Grease septage shall be diluted at least 1:1 from its concentration when pumped with domestic
septage or water if land applied over perennial vegetation. This dilution shall be increased if crop damage
occurs. This dilution requirement shall not apply to the liquid portion of grease septage that has been adequately
treated to remove solids, fats, oils, and grease as long as crop damage does not occur.
The discharge path outside the permitted boundary on the south west corner of Field 5 appeared to be
inadequately diluted grease septage. Other apparent grease burns from discharge events were noted on other
disposal fields. Particularly on Fields 1 & 2 where I was informed that it occurred from diluted effluent.

All grease septage (including effluent) shall be adequately diluted/treated to prevent crop damage. Review
procedures & implement changes to ensure proper dilution. Ensure that only septage waste (per the
definitions in G.S. 130A-290) is being stored/processed thru the SDTF & land applied on the SLAS. Repair
/reseed the damaged areas.
ADDITIONAL COMMENTS
1. The required signs are posted and the access road is adequate.
2. Field markers appear adequate. Setbacks will be measured on the next site visit.
3. The area at the retaining wall area (where the truck cleanout box was located) has been cleaned up.
4. Continue efforts to improve the cropstand. An adequate rye crop will be needed for land application to occur
this winter. Remove any hay bales located along the field perimeters.
5. The submittal for the permit renewal is under review at this time.
6. Soil samples will be taken by the permit holder before the end of the year.

administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
