The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. This facility is permitted to receive, consolidate and transfer yard trash (as defined in 15A NCAC 13B
.0101(55)), collected by the City of Winston-Salem, for storage and removal.
2. The facility operates only during the city’s curbside leaf collection program, between October and February.
Typically, the bulk of operations are carried out in November and December of each year. Leaves collected
curbside are transported by city trucks to the facility, where they are consolidated into transfer trailers and taken
either to the city’s Forum 52 compost facility (permit #34-23), or to Wallace Farms compost facility (permit
#30-04). No composting takes place at this site.
3. The facility is protected by means of a chain link fence that surrounds the site. Two locking gates serve to
secure the site when it is not in use. The gates function as ingress and egress for city trucks transporting leaves
to and from the facility.
4. The north side of the site, beyond the asphalt paving, slopes up steeply and is wooded.
5. Proper signage was observed at the entrance gate.
6. The facility was found to be clean, with no trash or windblown litter observed.
7. A portable loading ramp is staged at the site to facilitate the loading of transport trailers
8. The facility was empty during this inspection. Mr. Geis stated that city crews would begin preparations for
opening the site as time was available in the weeks leading up to the start of the leaf collection program. He
explained that several loads of gravel are brought to the site and used to level areas where the old asphalt paving
had eroded, to prevent water from ponding in and around the leaf stockpiles. A small area in the center of the
site was observed to have ponding water leftover from the previous evening’s thunder storms. Mr. Geis
confirmed that leveling the site with gravel would eliminate this problem while leaf operations are under way.
9. Kudzu was observed to be encroaching on the site, overgrowing the paved area on the northern edge of the
asphalt, and near the center of the fence line along the street frontage. Mr. Geis stated that it had become a
problem that had to be dealt with continuously. He asked if there were any recommended means of eliminating
it completely, and was advised to contact the county’s agriculture extension office for technical assistance.
10. The 50’ buffer that is required to separate waste from all property lines had been identified using a dotted white
line. On the northern edge of the paved area, some of this white line had begun to be covered by the kudzu
growth. Mr. Geis stated that the kudzu would be cut back again prior to the start of leaf collection to ensure that
the buffer line is clearly visible for all city personnel using the site.
11. The site is sloped to direct surface water to the city’s storm drains.
12. Mr. Geis confirmed that the facility’s Permit to Operate and approved Operations Plan are housed at his office
and made available to staff for review prior to the start of leaf operations. He added that all records of incoming
and outgoing loads are also maintained at his office.
13. The tonnage records provided in the FY2017-18 Facility Annual Report were reviewed. The report indicated
that an estimated 22,055.84 tons of leaves were collected at the facility during the specified period. This total
represents more than twice the amount of leaf waste received at the facility during FY 2016-17.
