Hilltop Road Red Springs, NC 28377 PARTICIPANTS: Connie Wylie STATUS OF PERMIT: Active – permit expires July 14, 2022 PURPOSE OF SITE VISIT: Routine STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: 1. .0838(a)(15) Grease septage diluted at least 1:1 from its concentration when pumped with domestic septage or water if land applied over perennial vegetation. This dilution shall be increased if crop damage occurs. Grease burns were noted on Field 1 (front field). Also noted dead bare patches that appear to be damaged areas from older grease burns. All grease septage shall be adequately diluted to prevent any grease burns. 2. .0838(a)(18) Approved nutrient management plans shall be followed. Permit condition 2. This site shall be operated and maintained in accordance with the nutrient management plan submitted by Emily & Randall Cooper and approved by the Division of Waste Management. Field 1 has not been harvested in a timely fashion & has exceeded the required harvest height of 12-18 inches per the approved plan. Land application has occurred on this field. Significant patches of weeds were also noted in some areas. Land application shall cease on Field 1 until the field is harvested in accordance with the required 30 day wait after the last discharge event. Currently Field 2 (back field) is partly harvested (with hay still on the field) and part of the field has not yet been cut. Once all the hay has been removed from the entire field then Field 2 will be available for land application. Until this occurs no land application may occur at this site. Talked with Ms. Cooper by phone. 4. .0838(d)(3) The permit holder of a site where more than one septage management firm has been authorized by the Division to discharge septage shall submit a monthly report to the Division which shall include the required information for each discharge. Another firm other than Cooper Septic Tank Service is authorized to discharge on this site and has been using this site. This office has not been receiving a copy of the site logs each month as required by Rule .0838(d)(3). The last audit of 6/07/2018 mentioned this Rule requirement. Begin submitting a copy of the site logs monthly. Hay bales located along the field perimeters need to be removed. 2. The required signs are posted and the access road is adequate. 3. Field markers appear accurate however there is still a missing post at the front corner of Field 1 (130 feet from the centerline of the road and 500 feet from the trailer) – this was noted on the last two audits. 4. No apparent sign of land application outside the boundaries. It appears the approved setbacks are being met but were not measured at this time. 5. An odor noted on Field 1. Ensure that all discharge events are at a pH 12 or higher. 6. Excess vegetation is hiding some of the field markers along the back side of Field 2. Remove all excess vegetation from around all the posts so that they are clearly visible. 7. Soil samples shall be taken by the permit holder this fall. This site is managed as two fields. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
