OBSERVED VIOLATIONS:
1. .0844(b) All permitted septage management firms shall display decals or lettering on each side of every pumper
vehicle operated by the firm. The decals or lettering shall include the firm name, address (town name), phone
number and septage management firm permit number. All decals or lettering on the pumper vehicle shall be no
less than three inches in height and plainly visible. Identification shall be permanently attached (no removable
signs).
The permit number was less than three inches on both sides of the pump truck. Correct the permit number
on both sides to three inches or more in height.
The firm name, phone number and town name were in compliance. However the phone number displayed was
336-991-5136 and needs to be the phone number listed on future applications.
A photo was submitted by email to show that the permit number was corrected to at least three inches in
height. Based on this information it appears that the truck lettering is now in compliance.

ADDITIONAL COMMENTS
1- Inspected pump truck in regards to issuing firm permit. Tag#YA131241, VIN# correct per the application.
2- Tank appears structurally sound & securely attached.
3- Outlet covers were on and no drips/leaks were noted.
4- No septage jobs shall be performed until the firm permit is issued by the Raleigh Central office. When
operation does commence begin maintaining the truck log to show the required information for each pump job.
5- Enclosed is a 2018 class schedule. Register for one of the remaining locations to obtain the training hours
required for permit renewal this winter.

or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
