PURPOSE OF SITE VISIT: Partial Facility Compliance Inspection.
STATUS OF PAST NOTED VIOLATIONS: None

None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS;
1 - This permit issued on January 24, 2017, is a Life of Site Permit as defined in N.C.G.S. 130A-294(a2).
Copies of this permit, the approved plans, and all records required to be maintained by the permittee must be maintained at the facility and made available to the Section upon request during normal business hours. 2 - The facility is permitted to receive solid waste generated within Chowan County, North Carolina. Waste
transported for disposal shall be sent to the East Carolina Regional Landfill in Aulander (Permit No. 08-03).
3 - The facility is secured by gates and fencing. 4 - Facility has signs posted with required information. 5 - Interior road in constructed of all-weather material.
6 - Tipping floor is kept very clean.
7 - Windblown materials are picked up multiple times during the day and checked at the end of the workday. 8 - Vector control OK. 9 - Leachate appears to be contained.
10 - Facility currently only has 2 operators. Lincoln Adams: expires, May 25, 2019 and Mike Winslow: expires, May
25, 2019.
11 - Records were available but not reviewed during this inspection. 12 - Fire suppression is handled by two freeze proof fire hydrants and a single fire extinguisher mounted on the wall. The
fire extinguisher is charged.
13 – Solid Waste is dumped directly into a compactor.
14 - Overall facility is operated well.
15 - Digital pictures taken.
