STATUS OF PAST NOTED VIOLATIONS:
.0838(a)(18) The site has been harvested and per conversation with Lynn Cummings the field rotation is being
followed. Georgia Love, regional agronomist, was contacted in regards to weed control.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. The rye has been removed from the site but weed control is needed (noted areas of weeds & vegetation other
than bermuda). Georgia Love visited the site on July 31st – follow her recommendations to reduce the
amount of weeds & also regarding the magnesium & potassium deficiencies (which will improve the
bermuda).
2. Noted spray paths on site. No sign of land application outside the permitted boundaries. No sign of heavy or
recent application. No odors noted. It appears that the approved setbacks are being met. Field markers appear
accurate.
3. The required signs are posted and the access road is adequate.
4. The renewal application was received and the submittal is currently under review at this time.
5. If you have not already attended a continuing education class this year, please register for one of the remaining
locations listed on the enclosed class schedule. (4 hrs firm, 3 hrs land app)
6. Soil samples shall be taken by the permit holder this fall.
