with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Inspected pump truck – tag#HV2112, VIN 1GDJ7H1J4RJ509792. This is a different tag & VIN numbers than
what is shown on the application submitted for the 2018 firm permit. Ensure that the correct tag & VIN
numbers are listed on the application submitted this winter for the 2019 firm permit.
2. The tank appears structurally sound & securely attached.
3. Outlet covers were on and no drips/leaks were noted.
4. The lettering is correct on both sides of the vehicle. However the phone number displayed on the truck is not the
phone number listed on the firm application. Ensure that the phone number displayed on the truck (704-
690-0327) is listed on the application submitted this winter for the 2019 firm permit. This was noted on the
previous audit of 6/24/2015.
5. Truck logs were not available for review at this meeting. Submit (Attn Connie Wylie) by fax [phone])
or email to [e-mail] a copy of the last three months of truck pumping logs.
6. If you have not already obtained the required 4 hours of continuing education this year then register for one of
the remaining locations listed on the enclosed 2018 class schedule.
