The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Met with Karry to gain access to the facility. Discussed the current non pumper status (the firm permit was not
renewed in 2017 or 2018). Karry stated that he would submit the needed form & fee. Discussed the upcoming
permit renewal and submittal for the SDTF permit – this permit expires December 31, 2018. Enclosed is a copy
of the notification letter which was issued regarding the SDTF permit expiration. Gave Karry an example
treatment log for reference. Site drawings & the operation manual for the waste process will be reviewed during
the upcoming SDTF permit renewal. Karry provided another mailing address for future use.
2. The tanks appear structurally sound. No leaks noted. The approved setbacks are being met. The required “No
Trespassing” sign is present on the gate. The required 5’ cleared radius (free of excess vegetation or debris) is
present around the tanks. Currently two firms are authorized to bring septage to this facility for treatment &
disposal.
