• Landfill operations contractor loaded trucks with CCR contaminated crane and truck mats to be disposed of in an offsite landfill (Republic Upper Piedmont Landfill in
Rougemont, NC).
• Truck drivers were given the wrong manifests and told to deliver material to Piney
Hill Acres LCID Landfill in Kernersville, NC.
• Two trucks delivered and dumped CCR contaminated wood material in LCID.

• Submit Environmental Event Report to DWM to document the incident

5. Duke Energy Follow-Up Actions:
• Landfill Operations Contractor went to LCID Landfill to oversee excavation of the CCR
contaminated material from the fill.
• Trucks were sent back to the Dan River facility for verification before being routed to the
correct landfill (Upper Piedmont).
• Moving forward, only one individual from the Landfill Operations Contractor company will
oversee offsite transport and disposal activities. That person will work directly with Duke
CCP Environmental Field Support to ensure the correct manifests are used to dispose of
the material in the correct facitlity.
