OBSERVED VIOLATIONS:
1) The letters for the Septage Management Firm permit number were less than three (3) inches in height. This is a
violation of Rule .0844(b) that states “All permitted septage management firms shall display decals or lettering on each side of every pumper vehicle operated by the firm. The decals or lettering shall include the name, address (town name), phone number, and septage management firm permit number. All decals or lettering on
the pumper vehicle shall be no less than three inches in height and plainly visible. Identification shall be permanently attached (i.e., no removable signs).”
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

1) All required lettering was on both sides of the pump truck. The letters for the Septage Management
Firm permit number were less than three (3) inches in height. The letters for the Firm Name, Town and
State, and phone number were all three (3) inches or greater in height. Pictures were emailed on
5/9/2018 showing that the letters for the Firm permit number had been increased to greater than three
(3) inches in height. See pictures below.
2) Tank made of metal and appears to be structurally sound.
3) Tank bolted to the frame of the truck.
4) Valves appear to be in working order.
5) No leaks noticed.
6) Cap present on the outlet port.
7) Talked about truck logs. Had paper copy of required truck logs.
8) Pictures emailed on 5/9/18 showing the letters of permit number to be greater that three (3) inches in
height. Thank you. Truck in compliance now. Permit issued on 5/10/18. Permit expires on
12/31/2018.
