ADDITIONAL COMMENTS
1. On November 11, 1991 ReUse Technologies submitted to the Solid Waste Section a request to fill part
of a 32.72 acre tract parcel ID# 387500004570. On November 4, 2004 the Section was provided with
a Closure Notice for the site.
2. Per the Groundwater Corrective Action Plan for the Former Swift Creek Coal Combustion By Product
(CCB) Structural Fill, CCB0057 approved by NCDEQ – Division of Waste Management on March 12,
2015, corrective action onsite activities began full time on April 24, 2017. The project raised the base
elevation and shrunk the total footprint of the structural fill. A synthetic cap system was installed with
2 feet of soils and vegetative cover. The construction activities included in the Corrective Action Plan
were completed in January 2018. The Construction Quality Assurance (CQA) for the project was
received by the Solid Waste Section on March 14, 2018.
3. A full history of the site, including the Monthly Project Updates for the Swift Creek Corrective Action

4. The final slope includes a bench approximately at the midpoint of the slope to slow storm water and
reduce erosion. The slopes are well vegetated but may require additional seeding to maintain
effective vegetative cover.

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

5. Small rills were visible at the time of the inspection. These rills were most evident on the southeastern
slope. Areas of erosion should be repaired as needed to protect the integrity of the cap.
6. Access to the site was controlled by a chain at the entrance off of Highway 301.
7. The northern portion of the site no longer has coal combustion by products but has not been
revegetated.
Flat area no longer has CCBs. All CCBs onsite are contained in the vegetated mound.
8. No coal combustion by products were visible at the time of the inspection.
