STATUS OF PAST NOTED VIOLATIONS:
No violations noted on the previous inspection conducted on 11/18/15.
OBSERVED VIOLATIONS:
NONE
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1) The required signage was present. 2) The access road had washed out near to the storage tanks. The road had washed out at a ditch due
to rainfall from Hurricane Matthew. Access road needs to be repaired.
3) No markers present for Field 3. Markers will need to be present prior to any septage applications to Field 3.
4) No recent applications noticed. No septage applications observed outside of the permitted
boundaries. 5) No application issues noticed.
6) Soybeans have not been harvested yet. A small grain crop will need to be planted if you plan to apply septage this winter. Some soil erosion noticed in Field 2. See picture below. 7) No objectionable odors noticed today.
8) Setbacks appear to be ok. 9) Records not checked.
10) I collected a composite soil sample from each of the three fields.
