OBSERVED VIOLATIONS: NONE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 AREAS OF CONCERN AND COMMENTS: 1) The required signage is present at the entrances to the site. 2) The access roads are well maintained. 3) Markers present. 4) No septage discharges noticed outside of the permitted field boundaries. 5) No spot dumps or thick applications noticed today. 6) The rye had been planted and was up on Field 3 and most of Field 2. The rest of Field 2 had been planted. Cows were present on Field 1. Cows had been place on Field 1 since the field they were on had been flooded due to Hurricane Matthew. Cows are to be taken off of Field 1 soon. Fields are not permitted to be grazed. 7) No application at time of inspection. pH not checked. 8) No objectionable odors noticed today. 9) No change noted with setbacks. 10) Records have been submitted with the application for permit renewal received on 03/18/16. 11) The air vent at Hydrant #1 is still leaking. Air vent needs to be repaired so that it does not leak. Since the hydrant is at the edge of the field, it would not take much for a leak to flow outside of the permitted boundary. Septage discharge outside of the permitted boundaries is a violation of Septage Management Rules. 12) I collected a composite soil sample from each of the 3 fields. 0BSeptage leaking out of the air vent for hydrant #1. 1BSolids build up coming out of air vent.
