The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1. Currently, the Septage Land Application Site consists of Field A.1 (3.5 acres), Field A.2 (9.4 acres), Field B.1
(2.9 acres) and Field B (8.3 Acres) for a total of 24.1 acres. All four fields are established in fescue. The fields were re-measured during the previous inspection to account for new wells and house.
2. The site is permitted for land application of domestic septage (including portable toilet waste) and grease trap pumpings.
3. The site had accessible all-weather roads to the septage land application field covered under the permit. 4. Appropriate signage was displayed at the site.
5. All fields had disposal area boundaries that were well delineated with permanent markers.
6. No recent evidence of septage land application were observed.
7. No objectionable odors were observed.
8. The fields had recently been cut but the hay was still on the ground.
9. A septage land application log is required to be maintained for the fields. This log is separate from the pumper truck log. A copy of the log form containing all required elements is included with this report.
