Robinson, Haynes STATUS OF PERMIT: Active Issued: 4/4/2017 Expires: 4/4/2022 PURPOSE OF SITE VISIT: Routine Inspection STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: N/A The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS 1. The Septage Land Application Site consists of Field 1 (2.5 acres), Field 2 (9.7 acres), Field 3 (4.2 acres) and Field 4 (5.7 acres), Field 5 (5.0 acres), and Field 6 (5.0) acres for a total of 32.1 acres. Field 4 is inactive at this time until trees are removed and approval by the Solid Waste Section is given for use. Total usable acreage is 26.4 acres. 2. The site is permitted for land application of domestic septage (including portable toilet waste) and grease trap pumpings. 3. The site is located at the intersection of Highway 18 and Glade Valley Road and has accessible all-weather roads to the septage land application fields covered under the permit. 4. Appropriate signage was displayed at the entrances to all fields and vegetation cleared. 5. The Fields 1, 2, 5 and 6 disposal area boundaries were well delineated with existing permanent markers at the time of inspection. Field 3 was marked with permanent disposal boundary markers with the exception of a small swale area at the northwestern corner and the southeastern corner which will need to be delineated, this was observed during the previous inspection and discussed during this inspection. The markers need to be replaced as soon as possible. Field 4 is still currently inactive. 6. All septage discharges appear to have been made within the disposal area boundaries. 7. No evidence of standing liquid or pooled waste was observed. The fields needed to be harvested soon. 8. Fields 5 & 6 need weed control. The nutrient management plan stipulates re-seeding and/or weed control whenever the groundcover by fescue is less than 80%. 9. No objectionable odors were observed.
