PURPOSE OF SITE VISIT:
Follow-up Inspection
STATUS OF PAST NOTED VIOLATIONS:
RESOLVED: 15A NCAC 13B .0302(2) states: “A facility shall only accept wastes which it is permitted to receive.” All
unacceptable waste, including the stockpile of horse manure and the incidental pieces of plastic and metal trash
found in the unprocessed wood waste pile, had been removed from the site. The manure had been taken by Jason
Carter, P.O. Box 441, King, NC, Stokes Co.; Farm # 710805; phone: 336-983-5489. Mr. Carter has used the
manure for land application on his fields.
OBSERVED VIOLATIONS:
None.

ADDITIONAL COMMENTS
1. Based on the recordkeeping discrepancy noted during the previous two inspections, a request to revise Section
L of the approved Operations Plan was submitted to Permit Engineer Ming-tai Chao for consideration. The
revision was reviewed and approved, and should resolve future issues with regard to records inspections.
2. All unacceptable materials found during the previous inspection on June 5, 2018 had been removed from the
site. (See Status of Post Noted Violations section of this report for additional information.) No other
unacceptable waste was observed during this inspection.
3. Extensive grading work had been done at the facility since the previous inspection on June 5, 2018. No ponding
water was observed in or around the operations area and storage areas. The site appeared to be properly sloped
to drain as designed. No evidence of erosion was found.
4. The facility appeared to be clean and well maintained. No windblown litter was observed.
5. No maintenance work had been performed on the sediment traps since the previous inspection on June 5, 2018.
Mr. Teague stated that he would begin work to clean out the ponds now that he had finished addressing the
grading issues. Ensure that drainage ditches, check dams, sediment traps and berms are cleaned and
repaired now, and maintained regularly hereafter, so that they conform to the design shown on the
approved facility site plan (dated October 13, 2009).
6. Mr. Teague had improved the access roadway system at the site as the grading work was taking place. During
this inspection, internal roads were found to be in very good condition and adequately sized to allow vehicles
and equipment to maneuver safely. A water truck was observed spraying the access roadways during the
inspection to control dust at the site.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
