FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. For issuance of the initial septage management firm permit - inspected pump truck, tag#YA145471, VIN 5PVNV8JM054S51182, 2500 gallon capacity. The tank appears structurally sound & securely attached. Outlet covers were on and no drips/leaks were noted. The lettering was correct on both sides of this vehicle. Informed the Raleigh Central office that the vehicle is in compliance at this time. 2. No septage pump jobs shall be performed until a firm permit is issued by the Raleigh Central office. When operation commences then begin maintaining the required information for the truck log in regards to each pump job. Discussed that attendance at a continuing education class is required before the end of the year (for firm permit renewal this winter). Mr. Babbitt indicated that he planned to attend the Greensboro location in late October.
