Subject:Approval of Permit #3419-TP Operations Plan Modification, DIN 28818
Dear Mr. Mayhew:
The Solid Waste Section received your request of an approval of a modification to the Operations Plan (DIN
22910) regarding the change of frequency to record/document waste amount received at the Solid Waste
Treatment and Process Facility owned by Lowder Recycling And Disposal, LLC, Permit No. 3419-TP-
2014. According to the revised Section L of the Operation Plan (DIN 28817) attached to your e-mail message
dated June 27, 2018, the inflow and outflow of wastes at the facility will be recorded in tons annually instead of
weekly.
The PERMIT CONDITION NO. 20 of the current Permit to Operate for the facility, issued February 02, 2015
(DIN 22911) requires an annual report of the facility activities for the fiscal year July 1 to June 30 being
submitted to the Solid Waste Section by August 1 of each year on the forms provided by the Solid Waste
Section; therefore, your request will not trigger an permit action at all, but be considered as an administrative
correction at your end. Your request hereby is approved, and this approval is also considered that Action Item
No. 5 stated in the June 5, 2018 Facility Compliance Inspection Report for the facility is resolved. Please place
the approval documents to the facility operating record.
Please be advised that this approval is not eliminated your responsibility for properly completing the requested
annual report. Additionally, please make all necessary efforts to resolve the other listed action items in the June
5, 2018 Facility Compliance Inspection Report for the facility. If you have any questions of waste operations at

Dear Sir,
In connection with the June 5, 2018 Facility Compliance Inspection Report (attached) performed by Susan Heim,
I am responding to Item 5 in the Additional Comments section of the inspection report which cites Section L of
the Lowder Recycling and Disposal Operations Plan, Permit No. 34-19TP. Section L indicates our record keeping
for inflow and outflow of yard waste and land clearing waste will be recorded in tons weekly. With the extensive
work effort required to maintain such record keeping on a weekly basis, we would like to amend the Operations
Plan to indicate such record keeping would be provided on an annual basis in connection with the Treatment &
Processing Facility Annual Report as of June 30 of each year. Attached is the proposed text for the revised Section L.
We will appreciate your consideration of this modification of Section L to allow us to maintain an efficient operation.
