(office only) STATUS OF PERMIT: Active Issued: 9/27/2013 Expires: 8/31/2018 PURPOSE OF SITE VISIT: Permitting Inspection STATUS OF PAST NOTED VIOLATIONS: N/A OBSERVED VIOLATIONS: N/A The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS 1. This facility currently is used only to discharge septage into the sewer line to the city of Greensboro. The facility also contains a spill containment area. Any use of this facility other than other than discharging septage into the sewer line will require the submittal and approval of a permit modification prior to implementing any changes. The NC Solid Waste Section must be contacted immediately if the City of Greensboro revokes the discharge authorization for this location. 2. Two steel tanks were observed inside a low block retaining wall. It appeared that no piping was connected to the tanks and they had not been used. If the tanks are to be used to store septage then a permit modification will be necessary before use. These tanks were also observed during the previous inspection on 4/18/2017. 3. Setback requirements appear to have been met. 4. Appropriate signage was missing at the entrance to the facility and will need to be replaced. 5. All weather roads were maintained for access to the site. 6. The tanks appeared to be structurally sound and well maintained. 7. The area around the detention and treatment area was found to be free of spills and debris. 8. No odors were observed.
