No violations noted on the previous inspection conducted on 05/11/16
OBSERVED VIOLATIONS:

The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1) Signage present at the tree to the northeast corner of the site. Sign may need to be moved since the
fences have been removed from the site.
2) Access road to the site is ok.
3) Markers present. The location of the markers will need to be checked. Not sure that the markers are
in the right locations since the fences have been removed.
4) A septage streak was noticed outside of the permitted boundary as you near the site. It appears that
there was some septage still draining from the tank after an application was completed.
5) One spot dump observed. See picture below. The solids need to be spread out and not just stay in
the pile.
6) Unable to judge crop stand due to height of vegetation. Weed control needed. A lot of grasses other
than fescue observed. The field needs to be harvested. See pictures below.
7) pH of septage not checked. No application at time of inspection.
8) No objectionable odors noticed today.
9) Records not reviewed today.
10) I collected a composite soil sample from the field.
Spot dump noticed in the field. Spot dump noticed in the field. The solids need to
be spread out and not just stay in the pile.
