STATUS OF PAST NOTED VIOLATIONS:
No violations noted on the previous inspection conducted on 07/27/16.
OBSERVED VIOLATIONS:
NONE
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

AREAS OF CONCERN AND COMMENTS:
1) Required signage present along the path before you get to the site.
2) Access road ok.
3) Markers present.
4) No septage discharges noticed outside of the permitted boundaries.
5) No application issues noticed today.
6) Bermudagrass had been cut however, the cuttings remained on the field. The cut bermudagrass
needs to be removed from the field. Also, a small grain or ryegrass needs to be planted as soon
as possible so that you will have a winter crop to apply to. Without a winter crop, you cannot
apply during the winter months.
7) No application at time of inspection. pH not checked.
8) Setback distances need to be checked.
9) Application for permit renewal received on 08/03/15.
10) I collected a composite soil sample from the field.
The bermudagrass had been cut but the cuttings remained on
the field.
